Step 1: Find
Use the map locator below to find your practice in our system. Please keep in mind that many practices have similar names, so it's best to search by city and state as well as your practice's name.
Can't find your practice? Contact our team for assistance.
Step 2: Claim
Once you've found your practice, click on its name to go to your Practice Profile. Then click the bright red "Claim This Profile" button at the top of the page. This will take you to our Claim Form where you can complete your claim in just a few seconds.
